General Description of the Job Class

Coordinates and performs nursing care for patients in alternative settings. Collaborates with other members of the health care team to provide integrated clinical services.

Duties and Responsibilities of this Level

Provides skilled nursing care to patients including, but not limited to: Patient assessment and reassessments. Patient and caregiver teaching. Development, revision and implementation of the Plan of Care and Plan of Treatment. Implementation of physician orders. Monitoring patient response to interventions Response to emergency patient needs. Psychosocial support. Demonstrates knowledge of growth and development, and demonstrates clinical competency in care of specific patient populations assigned. Completes all required patient care documentation and reports accurately. Submits all required patient care documentation and reports per policy. Communicates to supervisor, physicians and other health care providers any changes in the patient???s condition and assists in resolving identified problems. Takes professional responsibility for self-protection during required work in potentially hazardous situations (i.e., handling of cytoxic agents, handling of blood and body fluids). Represents self and company in a professional and ethical manner at all times. Complies with all applicable company policies and procedures. Attends patient care conferences, inservices, departmental meetings, etc. as requested. Works effectively with patient care team to clinically manage the patient???s care. Maintains confidentiality of patient information. Displays a neat, clean, professional appearance at all times. Participates in Performance Improvement Plan. Promotes quality, comprehensive services to all customers through a team approach. Maintains an effective level of communication between and among staff. Encourages open communication and verbalization of opinions. Gives high priority to client satisfaction and anticipates client needs. Maintains confidentiality of employee and patient information. Exhibits an attitude which promotes harmony and goodwill in the workplace. Displays a clean, neat, professional appearance. Participates in Performance Improvement Program. Promotes quality, comprehensive services through a team approach. Performs other duties as assigned.

Required Qualifications at this Level

Education

Work requires graduation from an accredited BSN or Associate's Degree in Nursing or Nursing Diploma program.

All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and to complete the program within five years of their start date.

Experience

Two years current medical-surgical, critical care or home care experience required. Previous home care experience preferred.

Degrees, Licensure, and/or Certification

Must have current or compact RN licensure in the state of North Carolina. BLS required.
