General Description of the Job Class

HIM Associate employees work in the area of document management.

Document management staff are responsible for preparing, scanning, indexing, importing and quality assurance of medical record documents. With responsibility for the work flow in both the paper-based environment and the electronic medical record. They are responsible to communicate with physicians and their office staff, as well as other medical center staff routine.

Duties and Responsibilities of this Level

Utilizes basic medical terminology and medical record content knowledge to review and analyze electronic documents scanned at the point of care or imported into the EDCO Solarity or Hyland OnBase application to validate correct selection of MRN, document type and date of service. Assigns or corrects key word values as needed prior to marking for upload to the electronic health record.

Utilizes billing, regulatory and insurance knowledge to prepare, scan, and index revenue documents in Hyland OnBase to the assign correct patient and title values. Once assigned and indexed, routes the electronic version of the document to the appropriate Revenue team or work queue for processing.

Prepares and scans records into the document imaging system using EDCO Solarity, for permanent storage of patient information. Correctly identifies all demographic information, removes erroneous documents from patient record and feed documents through scanner.

Reviews for quality the electronic documents scanned at the point of service following indexing, using Hyland OnBase or EDCO Solarity to validate correct selection of document type, patient, CSN/encounter and date of service and makes corrections as needed.

Captures quality review and feedback in Hyland OnBase of any errors identified and corrected.

Performs other duties as assigned.

Required Qualifications at this Level

Education

High school or GED diploma required.

Experience

1 year of experience in Health Information Management, Document Management, or Healthcare Related Field required

Associate's / Bachelor's Degree in Healthcare, Business, IT, or Related Field can substitute for 2 years' experience

Degrees, Licensure, and/or Certification

N/A

Knowledge, Skills, and Abilities

Working knowledge of the health record, working knowledge of privacy regulations, Basic PC proficiency; Data analysis and research skills for error correction required; Microsoft Windows and Office Suite
