General Description of the Job Class
The Community Health Worker provides non-clinical outreach, care navigation, and community based support to help patients and families access health, social, and community resources.
Duties and Responsibilities of this Level
Care Navigation & Support
Assist clients in meeting care plan components and navigating health and social service systems.
Assist program participants with applying for public benefit programs, through sustained relationships and follow up.
Identify and document health related social needs and connect clients to appropriate resources.
Provide outreach and care management support to patients across community settings.
Direct clients to referral sites, and community resources as needed.
Make home visits and community based encounters to support care continuity.
Maintain accurate documentation of services provided, neighborhoods visited, and barriers encountered.
Community Engagement & Partnership Building
Build and maintain relationships with community organizations, faith partners, neighborhood groups, and advisory councils.
Support community based initiatives related to food security, housing stability, neighborhood safety, and whole person well being.
Elevate community voice by gathering feedback and sharing insights with program leaders to improve accessibility and trust.
Required Qualifications at this Level
Education
Work requires an educational background equivalent to a high school education.
Experience
Work requires one year of experience in a position involving patient or public contact. Training or experience in community health, social services or nursing support preferred. Valid North Carolina driver???s license with a good driving record required. OR an equivalent combination of relevant education and/or experience.

Knowledge, Skills, and Abilities
Strong communication and interpersonal skills.
Ability to build trust and maintain relationships with clients and community partners.
Ability to engage with diverse populations.
Basic documentation and data entry skills.
Ability to work independently in community settings.
Knowledge of social determinants of health, community resource networks, and trauma informed approaches preferred.
