General Description of the Job Class
Nurse Navigator is responsible for facilitating and assisting patients through the healthcare continuum. This role increases the efficiency and quality of that delivered to the patient. Take a lead role in coordinating the multidisciplinary approach to planning and providing care. Serves as a patient advocate and liaison between the patient/family members and the physicians, surgeons and other healthcare professionals. Delivers supportive care in a personal manner and ensures that patients/family members receive all the needed resources and services required.

Required Qualifications at this Level
Education
Bachelor's Degree in Nursing Preferred
Experience
Two years of Nursing Experience in the area of "navigation concentration," i.e. oncology, bariatrics, orthopedics, preferably in a lead role. Prior nurse navigator experience preferred.
Degrees, Licensure, and/or Certification
BLS
