General Description of the Job Class
The Clinical Social Worker works collaboratively with the Case Management team and the multidisciplinary team to provide support, information, and referral sources to patients and their families. The Clinical Social Worker assists with the discharge planning process.
Duties and Responsibilities of this Level
Required Qualifications at this Level
Education
Master of Social Work (MSW) required. Adherence to National Association of Social Workers' Code of Ethics.
Experience
Minimum 1 year hospital discharge planning experience required.
Degrees, Licensure, and/or Certification
CMSW and/or LCSW preferred, as per NCSWCLB guidelines.
