General Description of the Job Class

Supervise and coordinate staff to ensure provision of quality patient care and effective team performance to meet agency goals.

Duties and Responsibilities of this Level

Supervise and coordinate staff to ensure provision of quality patient care and effective team performance to meet agency goals in accordance with philosophy, goals, policies, mission and vision.

Accountable for clinical staff and provision of appropriate clinical services to patients within an interdisciplinary team.

Evaluate clinical practice of specified discipline and review of clinical records to ensure standards of care and quality of services are provided in accordance with Medicare, Medicare and JCAHO. Maintain current provisions of Medicare, Medicare and JCAHO.

Coordinate staffing patterns to meet team and geographical needs among teams; take referrals as necessary to ensure agency coverage.

Collect and analyze staff productivity data; monitor and ensure adherence to plan of care, care map guidelines and identified group resources. Serve in rotation as on-call supervisor, with home visit coverage as necessary to ensure patient needs are met.

Facilitate the team process including interdisciplinary communications; coordinate training of new team members.

Interview applicants and assist in hiring decisions; perform performance evaluations.

Participate in collaborative team efforts with other health system members and community groups as needed.

Perform other related duties incidental to the work described herein.

Required Qualifications at this Level

Education

Work requires completion of an accredited Bachelor's degree in a clinical field such as Nursing, Social Work, Physical Therapy, Occupational Therapy, Speech Therapy or a related field.

Experience

Work requires a minimum of four years of clinical experience, including supervisory experience in home health care.

Degrees, Licensure, and/or Certification

Professional Licensure or registration in a clinical field is required.
