General Description of the Job Class

The certified nurse midwife (CNM) is an advanced practice nurse who is responsible for the assessment, plan, implementation and evaluation of nurse midwifery care delivered to antepartum, intrapartum and postpartum, and newborn patients. The CNM performs physical exams, collects and documents data, manages labor and spontaneous delivery, conducts diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, and prescribes appropriate interventions and medications. The CNM practice includes primary as well as specialized care provided in multiple locations such as the birthing center, triage, Lincoln Health Center, and outpatient clinics. The CNM promotes safety and quality in clinical care, contributes to research and teaching, and provides leadership in the organization.

Duties and Responsibilities of this Level

Clinical Provider Provide direct clinical services and management of mothers and babies as defined in the standing orders, policies, procedures and protocols for the certified nurse midwife. Obtain patient health histories and complete physical examinations. Document findings noting pertinent normal and abnormal findings. Order and interpret appropriate diagnostic and laboratory tests. Explain the necessity, preparation, nature and anticipated effects of procedure(s) to patient, family, staff, and healthcare learners. Perform approved therapeutic procedures based upon patient???s clinical status. Document patient preparation and response to procedure(s). Formulate and implement prenatal, intrapartum, postpartum, newborn and interconceptional care as defined by the nurse midwifery service. Collaborate with physician and nursing colleagues and document in medical records and appropriate narrative summaries. Utilize prescriptive privileges as allowed by the North Carolina Board of Nursing. Document all prescriptions in patient???s medical record. Assess patient for change in status and institute appropriate interventions. Initiate necessary emergency interventions to stabilize patient. Educator Assess educational needs, plan, provide, and evaluate educational programs for nursing staff, interdisciplinary healthcare team, and community. Precept nurse midwifery students and healthcare learners. Provide patient and family education related to prenatal, intrapartum, postpartum, newborn and interconceptional care. Research Utilization and Investigation Utilize current research and evidence-based decision-making in all clinical practice. Incorporate clinical research findings in the development and implementation of standards of care. Support IRB approved clinical research/trials though a variety of activities including but not limited to: obtaining consent; ordering diagnostic tests; recording laboratory & diagnostic results; administering therapeutic interventions; and documenting and reporting patient outcomes including toxicities or adverse events. Perform and participate in quality/performance improvement activities and clinical research. Consultant Serve as an expert consultant to nursing, members of the healthcare team and to the larger community. Participate in and support accreditation, compliance, and regulatory activities of the organization. Professional Role and Development Role model advanced practice professional nursing through conduct, appearance, communication, mutual respect, leadership, ethical decision-making, critical thinking, and problem-solving skills. Demonstrate responsibility for professional practice through active participation in professional organizations and continuing education. Enhance the body of knowledge of nursing or area of specialization through exchange of ideas and knowledge in professional organizations, oral presentations, posters, research activities, and written publications

Required Qualifications at this Level

Experience

3 years of nursing experience

Degrees, Licensure, and/or Certification

Must have current or compact RN licensure in the state of North Carolina. Certification as a Nurse Midwife is required. BLS required.

Knowledge, Skills, and Abilities

Knowledge of professional nursing theory, clinical practice standards, normal maternal / fetal growth and development, and requirements of nurse midwifery practice. Able to independently seek out resources and solve complex problems. Able to communicate effectively with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others. Able to use sensory and cognitive functions to process and prioritize information, perform health assessments, treatment, and follow-up. Able to use fine motor skills. Able to record activities, document assessments & interventions; prepare reports and presentations. Able to use computer and learn new software programs. Able to provide leadership in clinical area of expertise and in meeting organizational goals Able to navigate the Hospital and DUHS to provide clinical expertise to specific patient populations Able to withstand prolonged standing and walking with the ability to move or lift at least fifty pounds. Able to remain focused and organized.

Distinguishing Characteristics of this Level

Functions independently, but in collaboration with the attending physician and interdisciplinary healthcare team, to provide midwifery services to patients. Self directed, responsible and accountable for clinical practice. May supervise or oversee work of others responsible for patient care. Provides continuity, compassionate, culturally sensitive care to patients, and models this for students and colleagues. CNM interests and responsibilities extend to the community level where they strive to improve maternal/child health from a patient- centered/family-centered approach.
