General Description of the Job Class

To support the development of Duke relationships with referring providers and affiliated programs that positions Duke as the preferred provider of care that cannot be managed in the community setting. Duties are defined based on the specific area of responsibility.

Duties and Responsibilities of this Level

Represent DCI and oncology services to community providers in assigned geographical regions in support of Duke Cancer Centers and the Duke Cancer Institute.

Represent Duke Cancer Centers and the Duke Cancer Institute to local and regional providers and promote a positive relationship regarding clinical and research activities and programs.

Facilitate seamless communication between oncology providers and community providers ensuring ease of access for patients and exchange of clinical information, by educating providers on MedLink, obtaining access for the practices, verifying referring provider data is collected and correct and Epic, setting up necessary meetings and visits between the physicians and begin the point of contact for all referring provider concerns.

Triage new patient referrals efficiently to ensure timely access to care. Work closely with Access Center, NPCs and Nurse Navigators to obtain all necessary information and provide timely follow up to referring providers.

Demonstrate comprehensive expertise and full understanding of Duke oncology service lines and physicians.

Consult with oncology providers and staff to obtain information on clinical activities to distribute to referring providers, including Continuing Education opportunities. Partner with Marketing and Communications to develop necessary materials based on this information.

Make regular visits with community providers to discuss oncology patient care at Duke, referrals, and new treatments, and thank them for existing referrals to ensure continued positive relationships.

Cultivate relationships with community providers.

Working with oncology leaders, manage the process for field visits for new providers as they establish their practices.

Identify and facilitate continuing education opportunities and coordinate sessions with referring provider offices.

Analyze data to identify at-risk referral patterns and restore relationships with community providers if necessary.

Identify new referral opportunities in the community by continually analyzing cancer cases in North Carolina and the referral patterns of physicians in their territory. Track new physician practices in their territories. Monitor leakage rates within Duke.

Independently make decisions required to manage a productive territory.

Document findings; provide market intelligence from the referral community.

Additional Duties Participate in departmental meetings related to physician outreach, program growth and patient access.

Advise supervisor as to the status of current activities and additional or expanded program requirements.

Make recommendations for solutions to problems identified.

Maintain liaison with other programs within the Duke and the Duke Cancer Institute to coordinate program business and to accomplish program objectives.

Perform other related duties incidental to the work described herein.

Required Qualifications at this Level

Education

BSN required. Specialty certification and/or MSN preferred.

Experience

Five years of experience of specialty specific nursing in a hospital, clinic, or community setting.

Experience in Nurse Navigation preferred.

Completion of National Nurse Navigation Training preferred.

Metric tracking skills.

Degrees, Licensure, and/or Certification

Must have current or compact RN licensure in the state of North Carolina.

Knowledge, Skills, and Abilities

Excellent written, oral and conversational communication skills to effectively work with diverse groups.

Ability to analyze, evaluate multiple solutions and solve complex problems using well developed critical and analytical thinking skills.

Excellent time management skills needed to prioritize among many competing priorities.

Attention to detail needed to operate accurately and effectively.

Mastery of Microsoft Office, especially Word and Excel, and use of health record for documentation and/or reporting. Ability to promote programs and services to the community.

Ability to maintain strict confidentiality.

Ability to promote and build teamwork and multidisciplinary care concept.
