Occupational Summary

The Service Coordinator is responsible for identifying, coordinating and facilitating client referrals to treatment resources including intensive outpatient, inpatient, residential care and partial day hospitalization programs.

Work Performed

Identify, coordinate and facilitate client referrals to treatment resources including intensive outpatient, inpatient, residential care and partial day hospitalization programs.

Maintain external liaison with community and professional referral sources to meet clients needs.

Assist clients and their families in understanding their insurance coverage; obtain pre-certification verification as appropriate; assist clients in accessing funding support for rapid entry into intensive treatment programs.

Responsible for sharing clinical assessment data related to students referred outside of program to insure smooth transfer of care.

Monitor, track and update information regarding referrals.

Gathers clinical data.

Maintains records for information concerning services rendered to be used for program evaluation.

Provide training to interns regarding assessment and referral process.

Provide brief support and instructional guidance to ensure referral success.

Follow-up with clients and clinics to assess referral efficacy and client satisfaction with referral experience.

Interface with clinical staff, physicians, and clients families to discuss progress and treatment status and/or the nature of services suggested or provided.

Consult with faculty and staff to facilitate referrals to outside treatment resources.

Provide outreach and developmental programming activities.

Perform other related duties incidental to the work herein.

Required Qualifications at this Level

Education/Training

Bachelor degree required

Must be licensed or qualify to be licensed in the State of North Carolina if applicable.

Experience

Two years of experience required

Skills

Strong organizational and communication skills are necessary.

Demonstrated experience and ability to work as part of a highly cohesive and interdisciplinary team in a university counseling center.

Demonstrates experience and expertise in systems consultation.

Has experience with and commitment to working with diverse clients, including individuals from ethnically/racially diverse backgrounds and individuals representing varying sexual orientations.

Ability to interface with computer technologies designed to enhance referral procedures.

Ability to maintain the highest level of confidentiality and use excellent judgment in working with student clients who are in need of CAPS services.
