Occupational Summary

Provide a variety of casework and group work social services to patients and their families when personal, social or environmental difficulties exist which predispose illness or interfere with obtaining maximum benefits from medical or psychiatric care.

Work Performed

Interview patients, their families and other concerned parties to obtain information about personal, social and environmental history; make home visits for additional information as required.

Evaluate data gathered in terms of the medical or psychiatric plan of treatment, available social service programs and knowledge of casework or group work.

Participate in periodic staff conferences to present case histories; confer with other members of therapy team to develop comprehensive evaluations or plan pertinent therapy programs to provide maximum benefits for each patient.

Prepare and maintain a case record for each patient to describe the nature of social problems and services suggested or provided.

Assist patients and their families, through individual or group conferences, to understand, accept and follow therapy plans; provide services planned to improve or restore patients to optimum social and mental health.

Maintain liaison with community agencies; participate in area activities related to health and welfare services.

Accept on call responsibilities for after hours coverage if required.

Perform other related duties incidental to the work described herein.

Required Qualifications at this Level

Education/Training

Work requires a Master's degree in Social Work (MSW) from an accredited school of social work.

Experience

None required beyond education/training requirement.
