Occupational Summary
Manage, coordinate and participate in a variety of administrative, educational and/or clinical activities related to multiple community-based clinical and educational programs; prepare and manage grant proposals and related budgets; supervise staff and develop policies and procedures consistent with program objectives.
Work Performed
Assess need in at-risk populations in city, county or other geographic areas and develop or administer supportive community-based clinical and educational programs.
Establish viable coalitions among civic, community and health partners in specific locations to plan programs designed to meet chronic and communicable health care conditions, change health behaviors or establish new educational programs.
Plan, develop and implement goals and objectives of assigned programs in conjunction with other staff members within Community Health Division and/or associated partners including, but not limited to, Lincoln Community Health, Promising Practices and Durham Community Health Network; evaluate program objectives on a continual basis; implement changes to existing policies and procedures as appropriate. Participate in establishing and maintaining communication between Community Health Division and physicians, nursing personnel and other health care professionals regarding program objectives, policies and procedures. Determine fiscal requirements and prepare grant proposals including budgetary recommendations; manage and authorize expenditures of program funds for clinical services, health promotion/health education services, educational training programs for health professionals and paraprofessionals or other objectives consistent with the program(s).
Administer various personnel actions including, but not limited to, hiring, performance appraisals, disciplinary actions and other related activities. Compile and maintain records, reports and documentation of program activities for use in program evaluation.

Required Qualifications at this Level
Education/Training
Work requires graduation from an accredited degree program in a clinical, allied health, or community health related field.
Experience
Work requires five years of progressive experience in a related field with experience in developing and managing community-based health care services.
A master's degree in a related field may be substituted for experience on a 1:1 basis.
