Occupational Summary

The patient navigator provides navigation services across the cancer continuum to patients with identified barriers. Navigation services may include addressing barriers, providing emotional support, resource referral and care coordination. Patient navigation services begin at diagnostic resolution, into treatment and through survivorship and/or end of life. Additionally, the patient navigator functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for oncology patients and survivors. She/he is responsible for ensuring adult patients with an oncology diagnosis and into cancer survivorship receives quality and comprehensive services The patient navigator assist in the collection of data, building and maintaining community relationships , and connecting patients/families to resources to deliver a seamless model of access and care benefiting patients, participating physicians, and family members.

Work Performed

Demonstrates an ability to assess patient needs, make appropriate referrals face to face or via telephone to provide navigation and guidance to the cancer patient from diagnosis to survivorship. Navigate all criteria-based referrals through the healthcare system and as warranted, accompany the patient to appointments and/or diagnostic test.

Utilize and identify available resources to assist patients with overcoming barriers to receiving appropriate access to care and clinical trials.

Enhance access to therapeutic clinical trials and optimize communication with patients, families and investigators regarding therapeutic clinical trials.

Employ seamless processes for patient referrals to navigation services across the cancer continuum. This includes persons diagnosed with cancer from the community, through diagnostic resolution, into treatment and through survivorship and/or end of life.

Assist in the coordination and implementation of cancer educational workshops to inform the community of navigation services and other resources.

Assist in the development and pilot testing of a volunteer community navigation program for cancer patients.

Maintain a navigation services database that captures how patients were referred, the types of barriers patients faced, who provided the service and how was the multidisciplinary team informed of the patient's needs

Required Qualifications at this Level

Education/Training

Bachelors degree in a health-related field required.

Masters degree a health-related field preferred.

Experience

Three years of experience in patient education, nursing, planning, case management, or similar experience.

Skills

Excellent written, oral and conversational communication skills to effectively work with diverse groups.

Ability to analyze, evaluate multiple solutions and solve complex problems using well developed critical & analytical thinking skills.

Excellent time management skills needed to prioritize among many competing priorities.

Attention to detail needed to operate accurately and effectively.

Mastery of Microsoft Office, especially Word and Excel, multiple electronic databases and computer based information systems including eIRB, RedCap, and EPIC.
