Bio:
Sarah Tucker currently serves as the Nursing Program Manager for Population Health within Duke Primary
Care. Sarah leads 3 teams - adult Population Health Nurses, pediatric Population Health Nurses, and
Behavioral Health Care Managers. These teams vary widely in care delivered to patients, ranging from
Medicare Annual Wellness visits in adult populations, to complex care coordination and support in
pediatric patients, as well as collaborative care management of depression and anxiety in pediatric and
adult populations. Her areas of responsibility include hiring, design and coordination of onboarding,
direct supervision, development and implementation new programs, as well as continual process improvement
and monitoring of quality outcomes in each area. In collaboration with the Center for Geriatric Excellence
and the Duke School of Nursing, Sarah and colleagues presented a poster at the 2023 NICHE conference
demonstrating the academic-practice partnership to enhance nursing student's knowledge of caring for the
geriatric population.
Between 2013-2020, Sarah served as a bedside nurse on a 31-bed Cardiothoracic Surgery step-down unit at
Duke University Hospital where she progressed from new graduate nurse to Clinical Nurse III and eventually
Clinical Team Lead. Sarah was involved in several supporting initiatives while working in the inpatient
setting including Clinical Practice Council member, preceptor, member of various committees, and champion
roles. Sarah had the pleasure of being one of several Duke team members to represent our organization at
the 2019 ANCC Magnet Conference to participate in the celebration of achieving of Magnet status.
Sarah holds a Bachelors of Science in Nursing from The University of North Carolina Wilmington, a diploma
in nursing from Watts College, an Associates in Health Sciences from The University of Mount Olive, a
Bachelors of Science in Business Administration from North Carolina State University and is currently
pursuing her Masters in Healthcare Administration at The University of North Carolina Wilmington.
